HeyForm provides a web‑based interface for building conversational, that can be used as surveys, questionnaires, quizzes, or polls. Users can assemble a wide range of input types—including text, email, phone, picture choices, date pickers, and file uploads—and apply conditional logic or URL redirections to create dynamic, multi‑path experiences. The platform also offers integrations via webhooks and popular automation services such as Zapier and Make.com, allowing data to be sent to external tools.

The visual editor supports brand customization through themes, fonts, colors, backgrounds, and optional custom CSS for deeper styling control. Built with responsive design, forms adapt to mobile, tablet, and desktop devices, and analytics tools display completion and drop‑off rates while CSV export enables further data processing.

HeyForm is released under the AGPL‑3.0 license, can be self‑hosted on a user’s own server, and includes a free tier without subscription requirements. The project is maintained as an open‑source alternative to commercial form builders, targeting small businesses, creators, product, marketing, HR, support, and education teams that need full control over their form data.
