OpnForm

OpnForm provides a visual, no‑code interface for creating and managing web forms. Users can assemble unlimited forms with a variety of input types—including text, dates, URLs, and file uploads—apply conditional logic, and embed the resulting forms on any site or share a direct link. Submissions are stored, can be exported as CSV, and are accessible through built‑in analytics, email, Slack, Discord, or webhook notifications.

The platform is self‑hostable and released under the AGPL‑3.0 license, allowing anyone to run the core application on their own infrastructure without a subscription. A Docker‑based development setup is supplied, and detailed deployment guides cover both cloud and on‑premises installations. An optional proprietary Enterprise edition adds advanced features while keeping the core free and open source.
